I grew up doing catering gigs with my father who managed a great big city club. I have set thousands of place settings and could probably do it with my eyes closed. There are a few strict rules (forks on the left, drinks on the right), but otherwise it's sort of up for interpretation. This is the "Traditional American" setup. I imagine this will evolve at some point, but it's a good snapshot of the classic formality.
